| Come join the fun of playing organized non-competitive matches. For youth and adults, this sport offers the opportunity to make new friends, stay in shape and enjoy the beautiful outdoors. Team registrations will be taken with up to 14 players per team. Individuals, who do not have a team, may also sign up. |

 Registration Procedures
|
A. Please read all the information on this page!
B. Submit completed team registration form. (Individuals can still sign up.)
C. Adult Registration Fee: $200 per team
D.Youth Registration Fee: $75 per team
($35 for Rally Ball (includes racquet and t-shirt)
E. Payment must be paid in full at time of registration.
F. You can register online at www.activityreg.com
We reserve the right to place a team in the league where we feel the team is most competive.

League Format

| Adult Team Tennis |
| This will be a round robin format. Each team will play one match each week. Play will start at 6:00 p.m. at your assigned court. Nighly matches include: four singles matches and two double matches. |
| Youth Team Tennis |
This will be a round robin format. Each team will play one match each week. Play will start at 10:00 a.m. or 11:00 a.m., at your assigned court. Youth matches will include two singles and one doubles. (times subject to change.)

Team Rosters
1. Must be submitted concurrent with registration fee.
2. All information must be filled out completely.
3. All players must be listed on the team roster. (Individuals without a team must fill out an Individual registration form and we will form them into a team.
4. There is a minimum of 6 players on a team.
5. All players must have played at least 4 matches to be eligible to play in the tournament at the end of the season.

| Rules and Regulations |
| All matches and tournaments of this association shall comply with the rules and regulations of the Top of Utah Tennis Association, the Northern Utah Tennis Association, the Intermountain Tennis Association and the USTA. |
AWARDS
Adult Team Tennis
The first and second place teams will receive awards
Youth Team Tennis
The winning teams will receive a trophy and an invitation to the district playoffs.l
